A Therapy Puppy Dog (Basic Manners) class will begin on Tuesday, June 10, 2003 in Florence, MS.

This class is for dogs who are 3 months of age or older and owners who have an interest in doing pet therapy work. Older dogs WILL BE ACCEPTED.

Applications will be accepted through May 27, 2003.
Pre-Registration IS Required. Classes are limited.
Verification of vet care required.

To enroll in this class, please download the application packet by clicking below. This packet contains six pages.

The first page is an informational flyer regarding the classes, which includes our contact information. The second page is the application. The third page is the Rules and Regulations form. This form must be signed and dated and returned with the other forms and payment. The fourth page is the Vet Verification form. This form MUST be completed or the dog will not be allowed to participate in class. Most vets are happy to complete these forms. Your information will be verified by the instructor. This is to maintain a healthy environment for all dogs attending classes.

All three of these forms (pages 2-4) need to be completed entirely and mailed, along with your check for $65 per dog made payable to Suzanne Sims, to the address listed below on or May 27, 2003 in order to be considered for enrollment in the Basic Manners class.

The fifth page includes a list of things you need to know and need to bring to class. The sixth (last page) is a goals sheet. Please think about what your goals for you and your dogs are, write them down and bring to the first night of class.
